My name is Jan Burden and I am the Health and Social Care Development Worker at DCCS. My role is to work with Health and Social Care professionals to raise awareness of our service and work in partnership to improve services for Carers.

Bearing in mind that one in ten people are Carers, there could be a great many of your patients who are caring for someone. Evidence shows us that most people don’t see themselves as a Carer because it can take up to two years for someone to acknowledge their role. We feel it is important for health professionals to work in partnership with Carers as they know and understand the patient better than anyone else.

We can;

  • Provide Carer Awareness Training for practice staff
  • Help to organise Carer Awareness events at your surgery, which could help you to identify and support Carers in partnership with us.
  • Develop links between practice staff and outside agencies
  • Provide Carer information, such as posters and leaflets (with or without a leaflet dispenser)
  • Provide a quick and efficient way for you to refer Carers to us by using the Carers Prescription Pad
  • Encourage Carers to enter onto your practice's Carers Register

The Carers Trust – has teamed up with the Royal College of General Practitioners (RCGP) to publish a guide "Supporting Carers" an action guide for general practitioners and their teams to help them support Carers. The document can be downloaded at,805,PP.html

To provide a consistent and dedicated service our Carer Support Coordinators work on a patch-based system which enables continuity. We can provide you with an up-to-date contact list of our Support Co-ordinators showing the areas that each worker covers.
